Full-time (scheduled 72 hrs or more per pay period) Job Definition Job DescriptionSCOPE OF POSITION The Patient Coordinator provides services for all sleep center patients in order to improve coordination of care and testing and increase treatment compliance in an effort to maximize the overall patient care experience.
The Coordinator makes patient contact starting with generation of the referral and managing the entire patient experience through to its conclusion.
Management of the patient experience is defined as a partnership type relationship that assists patient understanding of the testing procedure, insurance requirements (copays, coinsurances, etc.
), follow up requirements and treatment with the goal of increasing patient compliance and satisfaction.

Responsibilities:
1) Admitting Patients:
Register Patients, obtaining appropriate documentation, insurance, co-pays, assists Clerical Associate as needed - answering telephone, scheduling or rescheduling patients.
2) Clinic/Visit:
Room patients, obtain vitals, obtain patient history, Perform CPAP downloads from home equipment for compliance; schedule return appointments; Acts as a liaison between the patient, home care company and sleep center to ensure continuity of care as needed 3) Home Sleep Studies (HSTs):
Program device and education the patient on application; Downloads device upon test completion or overnight; Cleaning of device; Acts as a liaison between the patient, home care company and sleep center to ensure continuity of care as needed.
Assists with questions and needs to the appropriate entity 4) Hospital:
Prepare sleep screening devices for inpatients STOP BANG; obtaining orders from physician; patient education prior to sleep testing.
5) Quality:
Assists tracking scoring and interpretation progress to be sure that studies are interpreted in a timely manner as defined by QA indicator; Follow ups as necessary.

EDUCATION AND EXPERIENC High School Diploma or equivalent Clinical experience with interaction of patient care One year experience in sleep lab services - testing, visits, etc.

Estimated Salary: $20 to $28 per hour based on qualifications.
