POSITION
Summary: Nurse II is considered the journeyman level of staff nursing within VHA. The staff nurse at this grade/level has developed and acquired the clinical experience and knowledge to be proficient in nursing skills, practice, and procedures and typically has developed a specialized nursing practice. The staff nurse at this grade/level manages clinically complex patients and situations related to patient care assignments, as well as clinical and administrative processes affecting the unit/work group/area. He/she is able to function independently, is self-directed in clinical practice and career development, and is actively involved in the functions and activities of the unit/work group/team. It is expected the nurse at this grade/level provides safe, effective, quality care to patients and possesses the management skills necessary to function as charge nurse. This nurse is responsible for guiding, directing, and appraising the performance of support staff and providing education for support staff, students, and new employees.
The RN uses the nursing process to implement patient care for self and others. He/she models the interdisciplinary team approach to patient care and involves others in this process. He/she provides education to patients, families and significant others; administers medications and treatments; participates in the quality improvement process; and is attuned to the needs of adult and geriatric patients. The RN is responsible for practicing in accordance with and adhering to regulations such as the Nurse Practice Act, VHA policies and procedures, and the standards of outside accrediting bodies.
The Clinical Care Manager is a registered nurse (CCMRN) assigned to outpatient primary care clinics with a patient caseload. S/he has significant collaborative responsibilities in chronic disease management. The CCMRN assists in the implementation of the Medical Home Builder model. The individual assists in the use of Chronic Disease Care Model with a focus at the practice level on patient self management support and integration of Chronic Model Components.
The CCMRNs work under the indirect supervision of the primary care providers and are responsible for direct patient care that includes assessment, planning, patient education, and disease management. The CCMRN manages a chronic disease case load and monitors patient's conditions, reviews and provides education regarding medication regimens, and provides of extensive counseling and motivation to help patients adhere to treatment regimens. CCMRN''s also function as needed as relief Head Nurses, program developers, and group clinic leaders.
Tour of Duty: Primarily Administrative (Monday - Friday, Day Shift) with flexibility required to work occasional early/late day shift tours as well as occasional weekends. Relocation Expenses: Relocation expenses are notauthorized for this position.
BASIC REQUIREMENTS:

Full unrestricted Licensure, Certification, or Registration required
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
EDUCATION/EXPERIENCE REQUIREMENTS:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor''s degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master''s degree in nursing (MSN) or related field with a BSN and no experience.
Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master''s degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.
Nurse III - Master''s degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.
GRADE DETERMINATIONS:
Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires periods of walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials, and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
