Clinical Pharmacy Specialist (Anticoagulation Clinic)

Functions include, but are not limited to:
Develops and maintains an in-depth knowledge of antithrombotic/anticoagulation management including pharmacotherapy, laboratory monitoring, and relevant medico-legal aspects of anticoagulation management. Manages the global and daily administration of the medical center Anticoagulation Clinic including clinic consults, home anticoagulation consults, database management, laboratory compliance of point-of-care monitors, individual clinic patient anticoagulation management, quality review, and clinic personnel training. Develops professional relationships with non-pharmacy professional personnel, and projects the highest level of pharmacy practice and competence. Monitors patient therapy utilizing advanced skills in physical assessment, pharmaceutics, pharmacokinetics, pharmacodynamics, and interpretation of laboratory and other diagnostic parameters. Reviews and evaluates prescribed drug therapy daily for appropriateness, effectiveness, cost-benefit, adverse drug reactions, allergies, drug-drug and drug-disease interactions, and communicates and documents recommendations to providers and pharmacists as necessary. Appropriately renders pharmaceutical care to Anticoagulation Clinic and Primary Care patients and documents care in the medical record. Reviews the patient medication profile prior to dispensing drugs and medications. All inpatient/outpatient medication orders are checked to include patient information, correct medication, dose, route, and frequency of administration. Orders are checked for patient allergies, drug interactions, incompatibilities, duplication, and priority. Identifies, interprets, and resolves prescribing and dispensing issues including pharmacotherapeutic and formulary issues. Documents interventions as necessary. Fills prescriptions by selecting the correct drug and dosage form, reducing the drug from bulk to the quantity prescribed, compounding when necessary, then packaging and labeling the drug in an appropriate manner. Supervises and trains pharmacy residents and students in anticoagulation clinics. Acts as a liaison between the clinical pharmacy managed Anticoagulation Clinic and medical center professional and administrative personnel. Actively assures that policy and procedures of the Service and medical center are followed. Assists the Chief of Pharmacy, Associate Chief of Pharmacy, Clinical & Educational Services, and pharmacy supervisors in developing policies and procedures. Consistently responds to patients, family members, caregivers, visitors, and co-workers requests for questions or assistance in a prompt, timely, and cooperative manner. Consistently communicates with all customers in a courteous, tactful, and helpful manner. Provides oral and written material promptly when asked and is always responsive to the customer's needs. (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. English Language Proficiency. GS-13:
(a) Experience. This position is above the full performance level for Licensed Pharmacists. 1-year of experience equivalent to the GS-12 level is required. This experience would allow you competently demonstrate the following GS-12 level Knowledge, Skills and Abilities (KSAs):
1. Knowledge of professional pharmacy practice. 2. Ability to communicate orally and in writing to both patients and health care staff. 3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. 4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. (b) Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following:
designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs:
A. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. B. Expert understanding of regulatory and quality standards for their program area. C. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. D. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. E. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. References:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
