Registered Nurse - ED Discharge Planner

Emergency Department (ED), Discharge Planner demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The Registered nurse (RN) is a PROFICIENT nurse, who understand a situation as a whole because they perceive it's meaning in terms of long-term goals. The ED Discharge Planner is administratively, professionally, and clinically responsible to the Nurse Manager with oversight from his/her Chief Nurse. The ED Discharge Planner is a self-directed professional who assesses, plans, implements, coordinates, monitors and evaluates patient care activities, while ensuring and facilitating the achievement of quality care. The ED Discharge Planner participates in interdisciplinary projects, committees, teams, etc., with the aim of improving outcomes in the management of complex patient care or processes of the team/workgroup. He/She is knowledgeable of clinical activities, education, quality improvement and resource utilization. He/She serves as a valuable resource to the varied community resources used to care for patients within the ED and acts as a liaison between patients in the ED and their medical team. The ED Discharge Planner is responsible for practicing in accordance with and adhering to regulations such as the Nurse Practice Act, VHA policies and procedures and the standards of outside accrediting bodies. This nurse demonstrates dedication to the nursing profession and represents nursing in community health professional activities. The ED Discharge Planner is responsible for ensuring discharge needs are identified and met through communication with patients/family members, care providers, medical providers, social work, and chart reviews. The ED Discharge Planner uses the nursing process and evidence-based practices to work collaboratively and effectively with the interdisciplinary team to ensure the patient needs were met after discharge from the ED. ED Discharge Planner has the skill, knowledge and experience to assume the following leadership roles:
Team Leader for planning, patient advocate, and patient educator. The ED Discharge Planner works as team leader in bridging the communication gaps between the medical care team, patients, family members and care providers within the interdisciplinary team to create a well-coordinated continuum of outpatient care. The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Experience:
-BSN with two to three years of experience as a Registered Nurse or MSN/Masters in related field with a BSN and approximately one to two years of experience as a Registered Nurse. - Demonstrated interpersonal skills, both verbal and written. - Dependable and ability to work a flexible schedule, including weekends, holidays and off tours - Computer literate - Demonstrated ability to provide education to patient, family and/or significant others Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. The population of male and female Veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. James H. Quillen VA Medical Center (JHQVAMC) is an interdisciplinary teaching facility, serving medical, surgical, and oncology patients. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, sitting, and computer usage. Transferring patients and objects may be required. May be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. May occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. With this position must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
