Discharge Planner - Registered Nurse

The Registered Nurse Discharge Planner is self-directed and works effectively in various areas of the hospital:
Inpatient Psychiatry, Medicine, Surgery, Emergency Room, PCU, and ICU. The Discharge Planner is responsible for devising and implementing discharge plans within the acute care setting. Discharge needs are identified through thorough interviews with patients/family members, care providers, medical providers, social work, and chart reviews. The Discharge Planner uses the nursing process and evidence-based practices to work collaboratively and effectively with the Interdisciplinary Team members. The following are among those in which the discharge planner will collaborate with to ensure a patient's discharge needs are met:
Patient, Family member/Home Care Provider, Acute Care Attending, Resident(s), Nurse Practitioner(s)/Physician Assistants, Staff RN, Social Worker, Nutritionist, Physical/Occupational/Speech/Respiratory Therapist, Chaplin, Hospice/Palliative Care Team, Home Oxygen/C-pap and Bi-pap Coordinators, Home Health Coordinators, Telehealth Coordinators, Primary Care Provider, Primary Care Manager, Utilization Management, Medical Coders and other Specialist/Consultants. The Discharge Planner has the skill, knowledge and experience to assume the following leadership roles:
Team Leader for inpatient and outpatient discharge planning, patient advocate, and patient educator. The Discharge Planner works as Team Leader in bridging the communication gaps between the medical care team, patients, family members and care providers within the interdisciplinary team to create a well-coordinated continuum of inpatient and outpatient care. The Discharge Planner is an effective advocate as she/he is found to be skillful in problem solving, communicating, influencing and collaborating. The Discharge Planner provides patient/family members/care providers with education on disease(s) or disease process with a focus on self-management, prevention and wellness, based on the patient's health care needs and choice goals. The Discharge Planner ensures all necessary medical equipment has been provided, (setup in the home if applicable) and patient has been educated on use. The Discharge Planner provides pertinent information to the Primary Care Manager on patient's hospital course, condition, medication (medication changes or additions), and other needs. The Discharge Planner provides pertinent information in a timely manner to other service partners such as:
Community Health Coordinators for home health care setup, Home Oxygen Coordinators for home oxygen setup, Telehealth Coordinators, and other Medical Care Team members involved in the patient's care both inpatient and outpatient to ensure understanding of the patient's medical condition and post discharge needs. The Discharge Planner ensures follow through of care has been achieved through post discharge phone calls to patient(s). The Discharge Planner reviews patient(s) chart reviews throughout their hospital stay. Through chart reviews the Discharge Planner ensures consults, labs, test, imagining and procedures are scheduled/completed and documented in a timely manner to avoid increase in Bed days of care. The Discharge Planner ensures the scheduling of Non-VA fee base consults and other follow up appointments as ordered or directed by the Attending provider. The Discharge Planner ensures medication reconciliation is completed prior to discharge, discharge medications are ordered, and post discharge appointments have been made. The Discharge Planner supports and enhances patient self-determination (patient autonomy) with respect to their personal preferences, health goals and beliefs. Pay schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Experience:
1- BSN and two to three years of experience as a Registered Nurse or Masters in Nursing or related field with a BSN and approximately one to two years of experience as a Registered Nurse. 2- Demonstrated interpersonal skills, both verbal and written. 3- Dependable and ability to work a flexible schedule, including weekends, holidays and off tours 4- Computer literate 5- Demonstrated ability to provide education to patient, family and/or significant others. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. James H. Quillen VA Medical Center (JHQVAMC) is an interdisciplinary teaching facility, serving medical, surgical, and oncology patients. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, sitting, and computer usage. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
